Ambulance Report
If you wish to raise an ambulance report with us, please send a secure email to the Social and Health Care Team (socailandhealthcare@oxfordshire.gov.uk) and as a minimum include the following information:
- The person’s details – name, address, date of birth, NHS number
- Contact information, including whether they can be contacted directly
- Next of kin or representative (particularly for those who can’t be contacted directly).
Please ensure that you check that the report you are raising is in relation to an Oxfordshire resident and that you have attached all the relevant documentation before sending it.
