Before you start the assessment

Please note:

To be eligible for support as an informal carer from Oxfordshire County Council, the person you support / care for, must be an ordinary resident in Oxfordshire. An ordinary resident is determined by which authority they pay their council tax to (e.g South Oxfordshire district Council, Cherwell District Council). Therefore, if they are not an ordinary resident in Oxfordshire, please contact their local authority to request a carers assessment or support.

It is advised that you prepare for your carers assessment beforehand and set aside time of up to one hour to complete it. Please use the 'Preparing for a care assessment' guidance (written by Carers Oxfordshire) below should you want more information about the carers assessment before starting your assessment.

Preparing for a carer's assessment 

Make sure you have the following to hand:

  1. Your NHS number - if you have one, (although you can proceed without it).
  2. Your GP's name, address and phone number.
  3. Details of anyone supporting you with your assessment.
  4. Details of the person you care for (name, address, date of birth, NHS number if known)
  5. Your email address, you will not be able to submit a form without an email address

This information will be stored securely and used to assist us in your assessment and offer of support. It will be used in accordance with the Data Protection Act 2018.
