What is the online self-assessment?
The online self-assessment is used to capture information about an individual living within Oxfordshire, who may need some support with some or all day-to-day living activities.
When should the self-assessment be completed?
You should complete this assessment if you are over 18+ years of age and are:
- looking for new help to be provided through us (Adult Social Care), or
- already receiving help through us (Adult Social Care) but your needs and/or situation have changed
- already paying for help but would like to check whether you can get support through the council.
What questions will be asked during the assessment?
The self-assessment is made up of two parts, and it may ask you for information that you may feel is not necessary, on a range of different areas of your daily life. This information is gathered to ensure we can offer and explore the best options to support you and meet your needs. .
The first part will ask questions that help us to understand why the assessment is being completed, as well as identifying any risks and areas of daily living that you may be finding difficult.
The second part of the assessment, should eligible needs be identified (under the Care Act 2014), will ask more detailed questions to help us understand how much help is needed and how much of this may come from unpaid family members, friends, neighbours, volunteers or other organisations.Do I need to create an account to complete the assessment?
Yes, you will have to create an account to complete the assessment form. Please note, you will need an email address in order to register and create your account.
Registering for an account means the assessment can be completed, saved and returned to at a later date, and then submitted once completed. The assessment doesn’t have to be submitted, but if it isn’t, Adult Social Care will not have visibility of your self-assessment form and therefore it will not be progressed any further.
What happens once the assessment has been submitted?
Once you have submitted the assessment, it will be passed to our Social and Health Care Team. From the information provided in the assessment, if there are any eligible needs identified, under the Care Act 2014, we will work with you and/or your relevant others, to identify the best way to support you. This could include short-term support (known as reablement support), assistive technology, equipment etc.
If eligible needs are not identified, Adult Social Care will provide the relevant information, advice and signposting to other organisations and support groups that may be able to help.
Please read these notes before you start:
- It is advisable to set aside some uninterrupted time of up to one hour to complete the Self-Assessment.
- The self-assessment can be completed by you (the person with care needs) or by someone acting on your behalf, for example, a friend, relative or health worker (with your permission or if the person contacting us on your behalf has the relevant Power of Attorney or Deputyship).
- Part completed forms can be saved and the option to print a PDF version will be made available, once the account has been created.
- If there hasn’t been any activity for 30 minutes on one page, the session may expire and the information provided that has not been saved will be lost.
- Please complete the form as accurately and comprehensively as possible, to enable us to fully understand your current situation. When describing any needs think about the worst day and how often needs significantly change or vary.
- Once a self-assessment has been submitted, changes to the document will not be possible via the online form. If changes are needed, a new self-assessment will need to be completed or wait for Adult Social Care to contact you.
- Providing your NHS number is helpful (the NHS number of the person the assessment is for), although the self-assessment can be completed without it.
- To enable us to provide the relevant support, we may need (with permission) to request further information from key individuals involved in your life e.g. health professionals, informal carer, family members etc. to validate information
- Information will be stored securely and used to assist us in your assessment and offer of support. It will be used in accordance with the Data Protection Act 2018.
