Before completing a carer's assessment
Please read these notes before you start
Please note:
- It is advisable to set aside some uninterrupted time of up to one hour to complete the Carers Assessment.
- You will be able to save part completed forms and print a PDF version should you wish to
- If there has been no activity for 30 minutes on one page then your session may expire and you will lose anything you have already filled in. Please make sure you schedule time when you are unlikely to be interrupted.
- Please complete the form as accurately and comprehensively as possible, to enable us to fully understand the impact your caring role has. The form will guide you to additional questions where appropriate.
- Throughout the assessment look out for the help icon which will give you additional information about a question.
Make sure you have the following to hand:
- Your NHS number - if you have one, (although you can proceed without it).
- Your GP's name, address and phone number.
- Details of anyone supporting you with your assessment.
- Details of the person you care for (name, address, date of birth, NHS number if known)
- Your email address, you will not be able to submit a form without an email address
This information will be stored securely and used to assist us in your assessment and offer of support. It will be used in accordance with the Data Protection Act 2018.

Other things you need to know:
- If you feel that the person you care for has unmet needs you can request an assessment for them. To do this go to the Oxfordshire County Council website or call the Social and Health Care team, option 1, on 0345 050 7666
- If you have concerns about the care they are receiving please call the Social and Health Care team, option 1, on 0345 050 7666
- It may be most appropriate for you and the person you care for to have a joint assessment. If you would like to arrange this please call the Social and Health Care team, option 1, on 0345 050 7666
- If you would like an independent person (an advocate who is not a friend or family member) to support you during the assessment process please call 0345 050 7666 or check guidance on the Oxfordshire County Council website.
